Rural Healthcare insight
Who can help an Illinois rural hospital with Rural Health Transformation Program funding?
A plain guide for hospital presidents, CFOs, and boards: what the program is, which grants reach your hospital, what the statewide assessment does and does not do, and where outside help fits.
The short answer
Most Illinois rural hospitals need help in the phase after ICAHN's digital readiness assessment: turning its findings into fundable projects, writing the applications, choosing technology independently, and carrying the reporting that follows an award. That help can come from in-house staff, ICAHN's own programs, or an independent advisor designated by the hospital. 618Today is one such advisor, based in the 618.
What the program is
The Rural Health Transformation Program (RHTP) is a federal program created by Public Law 119-21 in July 2025 and administered by CMS through the states, worth $50 billion nationally over five years. Illinois received a Year 1 award of $193.4 million, managed by the Illinois Department of Healthcare and Family Services (HFS). HFS has determined that 78 Illinois rural hospitals are eligible, 56 of them critical access hospitals.
How the grants reach your hospital
Illinois funded eight RHTP initiatives as ten grants. Only one, Hospital Transformation Planning, is a competitive application made directly by hospitals, and it carries the Grant Accountability and Transparency Act (GATA) obligations with it. The others reach a hospital through a sub-award or through a partner, such as ICAHN, IPHCA, a university, or your EMS Resource Hospital.
ICAHN, the largest sub-awardee of Illinois's first-year funding, administers three programs for eligible hospitals: Technology Transformation, Disease Prevention and Chronic Disease, and Rural Workforce Investment. Applications run through the state's Euna grants portal. Our RHTP page lists all ten grants and who administers each.
What the digital readiness assessment does, and does not do
ICAHN coordinates a digital readiness and needs assessment for all 78 eligible hospitals. It gives each one a current-state baseline: maturity scores, gaps, risks, and dependencies. By design it stops there. It does not recommend vendors, build technology roadmaps, or implement projects. Those decisions are left to each hospital and, in the assessment's own words, to "any designated advisors in a subsequent phase."
The next phase has a hard clock. Illinois HFS has set the deadline: Year 1 technology funds must be spent by September 30, 2027.
Where outside help fits
- Turning findings into a plan your board can approve: prioritized, fundable projects built on the assessment evidence.
- Writing and submitting applications: narrative, budget worksheet, and portal submission aligned to the program's requirements.
- Choosing solutions independently: telehealth, EHR, analytics, and AI options evaluated against your needs by someone who sells none of them.
- Governing AI before it is deployed: automation and AI are on the funding list, and in a hospital AI should support clinical and operational judgment, not replace it.
- Carrying the reporting: GATA obligations, progress reports, drawdowns, and audit documentation for the life of the award.
What to ask any advisor
- Do you resell software or take vendor commissions? (618Today does neither.)
- Will you stay for the reporting after the award, or only write the application?
- Who exactly will do the work, and have they supported RHTP applications before?
- Are the scope and price agreed in writing before work begins?
618Today has supported multiple Illinois rural hospitals with RHTP Hospital Transformation Planning applications against a single state deadline, each with a full deliverable set, and every application was submitted on time. 618Today Inc. is an independent advisory firm and an ACHE member. It does not act for HFS, CMS, or ICAHN.
Questions hospital leaders ask
Is my hospital eligible for Illinois RHTP funding?
HFS has determined that 78 Illinois rural hospitals are eligible, 56 of them critical access hospitals, and ICAHN publishes the full list. More than 30 of the eligible hospitals are in the 618 area code region of Southern Illinois and the Metro East.
When do Year 1 RHTP technology funds have to be spent?
Illinois HFS requires Year 1 technology funds to be spent by September 30, 2027, so hospitals that move quickly from assessment to a defensible plan will be ready when applications open.
Can an outside advisor work on our hospital's RHTP application?
Yes. The statewide assessment dataset is designed to be used by participant hospitals and any advisors they designate for the next phase, and many hospitals use outside help for planning, applications, and post-award reporting.
